Job Description:
JOB SUMMARYUnder the direction of the Director of Pharmacy, the Stem Cell Clinical Pharmacy Specialist will be responsible for the development, implementation, and maintenance of clinical pharmacy services for the cellular therapy program.
This includes providing evidence-based, patient-centered cellular pharmacotherapy management while serving as the subject matter expert for the multidisciplinary care team (including but not limited to:
hematology, stem cell transplant, and CAR-T therapies).
This includes treatment assessment of chemotherapy regimens and supportive care guidelines; providing recommendations to physicians and advanced practitioners; providing medication education to patients, physicians, and other healthcare members; being a pharmacy representative for the cellular therapy team and coordinate with other pharmacy personnel to ensure safe and timely mediation.
JOB RESPONSIBLITIESDirect development, implementation, and maintenance of clinical pharmacy services as related to the cellular therapy program.
Facilitates the development and implementation of treatment guidelines, protocols, formulary changes, and critical pathways as approved by appropriate committees.
Directs the clinical functions performed by pharmacists, technicians, and pharmacy interns/students.
Monitors and documents staff performance in area of clinical intervention and outcomes.

Review, select, and dispense safe, appropriate, evidence-based medication therapies for patients according to established policies, procedures, and protocolsVerify and assess all medication orders including but not limited to:
antineoplastic regimens, supportive care, symptom prevention and treatment, pain and palliative care, antimicrobial regimens, immunosuppression regimens, anticoagulation and graft-versus-host disease treatment, for clinical appropriateness based upon patient's current lab values, vitals, symptomology, and disease state(s)/comorbidities.
Actively participate in daily interdisciplinary rounds with stem cell transplant physician and/or mid-level practitioner Assist with therapeutic drug monitoring of immunosuppression regimens, chemotherapy regimens, anti-infectives as appropriate.
Participate in clinical documentation for all pharmacist interventions in EHR as well as Vigilanz.
Work with other departments to obtain insurance pre-certification.
Direct and supervise the work of pharmacy technicians to ensure timely medication delivery.
Provide education as appropriate to patients, caregivers, and members of the healthcare team.
Educate patients and caregivers at discharge to provide full instruction on administration and side effects of immunosuppressant agents, antimicrobial agents, and other pertinent post-transplant medications.
Provide general and patient specific drug information to patients, physicians, nurses, and other health care professionals.
Precept pharmacy students as assignedServe as the pharmacy representative for the cellular therapy team including stem cell transplant and related hematology aspects for EHR related questions.
This includes creating or modifying chemotherapy order-sets including preparative regimens and answering EHR questions for nursing/physician colleaguesBe competent in preparation and dispensing of sterile compounded products following guidelines outlined within USP 797, and USP 800, and able to supervise technician staff performing these functionsParticipate in departmental and hospital quality assurance programs with an emphasis on patient safety, and regulatory compliance.
This also includes participating in regular stem cell transplant meetings, P&T committee and other ad hoc groups as required.
Work with corporate pharmacy representatives for cellular therapy including hematology/stem cell transplant deptFulfill pharmacy requirements to maintain FACT accreditation for the cellular therapy program.
This includes but not limited to attending required meetings and maintaining expertise in areas of practice through continual learning, journal review and other educational activities.
Perform other duties as assigned JOB QUALIFICATIONS REQUIRED Experience-Minimum of 2 year's institutional oncology pharmacy experience or strong interest in hematology/stem cell transplant pharmacyEducation-Graduate of an ACPE-accredited School of Pharmacy Licensure-Active license with the Georgia State Board of Pharmacy PREFERRED Experience-Successful completion of hematology/oncology residency desired; 5-7 years health system experience with emphasis on clinical pharmacy positionsCertification-Board Certification in Oncology Practice (BCOP) preferredPay Range $47.
