00p The Cellular Therapy Post-Transplant Coordinator is a registered nurse and a critical member of the Cellular Therapy team, responsible for ensuring that patients receive comprehensive and seamless care following a stem cell transplant or CAR-T therapy.
This includes coordinating frequent post-transplant follow-up treatments and appointments, monitoring for complications, and providing education and support to patients and their families.
This position also requires coordination of financial and insurance-related matters.
The coordinator will work closely with the patient's multidisciplinary cellular therapy team and other healthcare providers and/or specialists to ensure continuity of care throughout the post-transplant period.
Additionally, the coordinator will provide emotional support, counseling, and assistance to accessing community resources for patients and their families as needed to help them navigate the post-transplant period.
JOB RESPONSIBLITIESMonitor patients for transplant and CAR-T related complications and coordinate appropriate interventions as needed:
This includes monitoring for signs and symptoms and requires the ability to recognize complications such as infections, graft-versus-host disease, CAR-T toxicities, and other side effects.
The coordinator will communicate with the patient's transplant team and coordinate interventions such as medications or additional treatments as needed.
Coordinate post-transplant treatments and follow-up appointments with patients and their families:
This includes coordinating appointments with the patient's transplant team within various areas including outpatient clinic and infusion areas.
It also involves coordinating appointments with outside facilities for needed follow-up testing and/or need for specialist referrals.
Complete prior authorizations for home specialty medications in a timely manner:
This includes taking a proactive approach in acquiring necessary specialty medications to minimize delays in treatment.
Evaluate transition of care needs and facilitates transition to various levels of clinical care settings.
This includes coordinating care within the inpatient setting, ambulatory care, outpatient clinic, and other outside facilities.
Arranges home care services as needed.
Collaborate with cellular therapy finance specialists and liaise with insurance case managers to ensure appropriate authorizations, timely utilization review, and maximize program reimbursementEducate patients and their families about post-transplant care and self-management for better adherence to care and improved quality of life:
The coordinator will provide education and support to patients and their families on topics such as managing side effects, infection prevention, and self-care after a stem cell transplant or CAR-T therapy.
Provide patients and their families emotional support and help navigate the healthcare system for access to resources and support services.
Participate in ongoing professional development and training to stay current on best practices and advances in the cellular therapy and cancer survivorship.
Participate in research and implementation of new educational information to remain current on programs, processes, and procedures that impact current training and developmental projects.
Maintain accurate patient records and ensure compliance with regulatory standards:
The coordinator will be responsible for maintaining accurate and complete patient records and ensuring that the patient's care complies with all relevant regulatory standards and guidelines JOB QUALIFICATIONS REQUIRED Experience-3 - 5 years Oncology/Hematology Education-BSN required or enrolled within 6 months of hire and BSN completed within three years from hire date Licensure-Georgia Registered Nurse License or a Registered Nurse License form a Compact State Certification-Basic Life Support (BLS) Certification Special Skills-Oncology/Hematology PREFERRED Experience->5 years Oncology/Hematology (Cellular Therapy/Stem Cell Transplant Education-MSN Certification-Advanced Cardiovascular Life Support (ACLS) Certification Oncology Certified Nurse (OCN) ONS Chemotherapy and Biotherapy Provider Special Skills-Oncology/Hematology/Stem Cell Training Pay Range $74,149.
92 - $122,107.
44Placement within the identified pay range is based on individual and market factors including, but not limited to, experience, education, credentials (including licenses and certifications), geographic location, market competition, skill set (including market availability of required skills), assigned/anticipated job tasks, and level of responsibility.
